WellFort Community Health Services needs your support to improve access to health care for non-insured persons in our community. We provide comprehensive primary care, preventive healthcare as well as health promotion programs connecting our clients to resources, supports and social services that addresses the social determinants of health like housing, food insecurity, etc.
We offer these services to populations in our community who traditionally face barriers accessing health services; Non-insured individuals (persons without a health card) being one of such priority populations. As an organization, WellFort is committed to removing barriers and improving access to care especially for people who are non-insured in Brampton and Malton. We need your support to do that. We know timely access to care is important to prevent ill health and to improve health outcomes of the most vulnerable in our community.
